Superior Health’s Social Determinants of Health (SDOH) Z code documentation guide. This guide was created to help support organizations collect ICD-10-CM Z codes specific to persons with potential health hazards related to socioeconomic and psychosocial circumstances (codes Z55-Z65). It was updated to include new codes released on Oct 1, 2023. The guide highlights Z codes that can capture Social Drivers of Health (SDOH), allowing for systemic data collection within electronic health records (EHRs). The guide also provides information on SDOH indicators that can be billed via claims data when captured in patient records using the highlighted Z codes.
