Trifold brochure to help educate patients about the grievance process.
The Forum of End Stage Renal Disease (ESRD) Networks Kidney Patient Advisory Council (KPAC) made The Patient Grievance Process Toolkit to help patients as they work through the Grievance System. We feel that a toolkit designed BY patients and FOR patients will help explain the grievance system to meet patient needs. The Toolkit could be a useful tool for both patients AND facilities. While most facilities and all Networks have a Grievance Process, this is the first time that process has been explained through the patient’s eyes.
We would like to offer guidance on ensuring that patients in your facility are aware of, and feel comfortable filing a grievance. Please take a moment to view the grievance policy guidance here.
View a new brochure for patients from Midwest Kidney Network. The brochure describes the role of the Network, how patients may file a grievance, and patient rights and responsibilities.
This poster and/or handout describes the rights and responsibilities of ESRD patients. It also provides contact information for Midwest Kidney Network and your State Survey Agency. Download the appropriate copy for your state below.
